These forms are updated frequently and new versions will be uploaded as they become available. Please check to make sure that you have the most recent version.
Please read the following descriptions carefully and choose the application that most closely fits your request. If you cannot find a form that fits your request, contact the appropriate Regional Funding Administrator. Applications can be completed online and submitted electronically to the appropriate Regional Funding Administrator.
Important: Give us as much time as possible to consider your request. It takes a full 120 days after a complete application is received to determine whether or not the request will be granted. Additional time is needed to process the request through legal, finance, and to order product, if necessary.
Form Download Instructions
We are aware of an issue that sometimes occurs when downloading forms on this page. We are actively working on a solution. In the meantime, the following steps will allow the forms to be downloaded:
- Right-click the form you would like to download
- Choose ‘save as’ and select a location where you want to save the form on your computer
- Open the form from your local machine. You may also contact us at GrantsEMEA@CookMedical.com for assistance in obtaining these forms.
- Do not attach files that total more than 10 mb to your email application. If your files add up to more than 10 mb, please contact your Regional Funding Administrator for alternate instructions for your files.

Charitable Programs
These donations are for programs that benefit the public, such as patient education, disease awareness, medical care for underserved populations, or events where proceeds are intended for charitable purposes rather than for healthcare education or individual patients.
Cook Product Donations for Charitable Use
These donations are for programs that benefit the public, such as patient education, disease awareness, or medical care for underserved populations. These donations are also available for events where proceeds are intended for charitable purposes, but the donations cannot be used for healthcare education or for individual patients.
Educational Grants:
Third-Party Educational Conferences
These grants are for use by educational institutions, professional societies, or other third parties to support educational conferences, seminars, or professional society meetings that they are organising. Cook Medical is not permitted to sponsor educational conferences or programs that are organised by healthcare institutions that restricts attendance to their own healthcare professionals who are not in training. (These sponsorships cannot be used for exhibit fees alone or other fee-for-service arrangements.)
Cook Product Donations for Third-Party Educational Use
These grants are for products that will be used in third-party educational conferences or programs for healthcare professionals. Cook Medical cannot control the program content. (These product donations should not be used for Cook Medical product training.)
Conference Grants for HCPs-In-Training
These grants are for an educational institution or professional society that wishes to support the educational advancement of healthcare professionals-in-training (HCPs-in-training), defined as medical students, interns, residents, and fellows. The requesting institution or society is responsible for selecting the beneficiaries of the grant. Some governments or institutions have their own requirements for making these selections.
Grants for Fellowships or Residency Programs
These grants are for use by structured fellowship programs within a healthcare institution for HCPs-in-training.
Product Donations or Grants for Research
Cook Medical is not currently accepting research grant applications.
